Fees & Additional Information

I am contracted with specific insurance providers* and offer self-pay options as well. If necessary, I can offer you a super bill for you to submit for reimbursement from your insurance company.  Please take time to contact your insurance provider to determine if reimbursement services are offered, and how it will apply to out of network expenses. 

Fees:

Individual Session: $95

Couples Session: $115

Sliding Scale: Please inquire for eligibility

Cancellation fee (under 24 hours notice): $50

Documentation: You will be required to sign documentation related to services. Informed Consent for Services, HIPPA and Privacy Practices, Electronic Communications and Social Media Acknowledgment,  and Telehealth Consent forms will be completed at the beginning of the first session.
